Essentials of Person-Centered Case Management: Winter 2023
Case managers are a critical part of any program or service in the housing, homelessness, and behavioral health fields when working with individuals and families who have been systematically marginalized. Quality, person-centered case management requires continuous development of abilities, experiences, and knowledge. Case managers are asked to be empathetic, practical, efficient, trauma-informed, enthusiastic, and mission driven, often with only a minimum of training and skill development. They are frequently expected to learn while they are on the job and often direct service work is prioritized over training.
During this course, we will cover the following topics and skills for practicing person-centered case management: race equity, Motivational Interviewing, trauma-informed care, Critical Time Intervention, working with people with lived experience, stigma, Harm Reduction, co-occurring disorders.
The course offers multiple interactive webinars to introduce new case managers to a wide range of concepts and encourage more seasoned case managers to recap or brush up on their skills.
Learning Objectives:
By the end of the course, participants will be able to:
- Explain to colleagues and supervisors the myriad of skills and abilities they need to support their clients
- Articulate their increased knowledge related to each topic, and how the subjects build on one another for a holistic perspective
- Identify the topics and subjects during the course that most interest them, and have a clear path towards further learning

Meet the Faculty:
Jon Cox, BS, has twenty-five years of experience working in homelessness, substance use, and supportive housing in the U.S. and United Kingdom. Jon focuses on housing and homelessness technical assistance and is a subject matter expert in recovery housing. He has provided technical assistance to numerous Continuums of Care, led the design and implementation of a diversion program with Ohio’s Balance of State Continuum of Care, and supports communities to increase recovery housing capacity and implement Medication Assisted Treatment (MAT) protocols within recovery residences. Steven Samra, MPA, has expertise and lived experience in substance use and mental health recovery, harm reduction approaches, promotion of lived experience as critical experiential knowledge, developing and leading peer advisory councils, peer leadership, cultural competence, criminal justice, and effective outreach and engagement for challenging populations. He serves in leadership capacities for multiple recovery, substance use, mental health, peer involvement, and homelessness focused initiatives.
Ken Kraybill, MSW, has worked at the intersection of healthcare, behavioral health, homelessness, and housing for more than 35 years. He is a Senior Trainer at C4 Innovations and is dedicated to improving the quality of care provided to marginalized and vulnerable people. Ken develops curricula and facilitates in-person and online trainings nationally on best practices including motivational interviewing, trauma-informed practice, trauma-informed supervision, outreach and engagement, recovery support, critical time intervention, and resiliency and renewal for care providers. Ken has a Master of Social Work from the University of Washington in Seattle. He is a member of the Motivational Interviewing Network of Trainers (MINT).
Bebe Smith, MSW, LCSW, is a clinical social worker, consultant, and trainer, and former clinical assistant professor of social work and psychiatry at University of North Carolina (UNC) at Chapel Hill. In her clinical and educational work, Bebe has focused on helping people with severe mental illness through interventions and strategies key to social work practice—family psychoeducation, psychotherapy, case management, and self-empowerment. She led a pilot of Critical Time Intervention in North Carolina and successfully championed the model for adoption by the state mental health system in 2014. Bebe graduated from Vassar College with a BA in English in 1984 and received a MSW from UNC School of Social Work in 1993. She worked as a volunteer in a psychiatric hospital in England in 1984 and received some of her best training as a graduate student at Club Nova, a clubhouse model program in Carrboro, NC. She was NASW NC Social Worker of the Year in 2012.
Elizabeth Black, MRC, LADC, has worked with and on behalf of marginalized populations for more than 15 years. She holds a master’s degree in Rehabilitation Counseling and is a Licensed Drug and Alcohol Counselor. Elizabeth serves as subject matter expert, trainer, and technical assistance provider for substance use, mental health, and recovery initiatives. Her areas of expertise include harm reduction, medication-assisted treatment, co-occurring disorders, and supporting systems change. Previously, she oversaw health and wellness initiatives statewide for the Oklahoma Department of Mental Health and Substance Abuse Services. In that role, her team received a governor’s commendation for their work which resulted in better health outcomes in Oklahoma’s most vulnerable communities.
Daisy M. Hernandez, MSW, native of Puerto Rico, has dedicated her clinical social work career to community mental and behavioral health organizations within under-served communities. Her professional experience spans the entire spectrum of substance use disorder (prevention through recovery) from peer roles to management. She provides training and technical assistance for mental health and substance use providers for the Praxis: Training for MA Addiction Professionals initiative.

Training Certificates:
Participants may earn a Certificate of Course Completion* by:
- Attending all eight, live webcasts (or watching the entire recording of each live session)
- Completing all required learning activities
- Scoring 80% or higher on each weekly quiz
- Completing the course evaluation form and the certificate request form
12 hours of continuing education credit will be awarded upon completion of this course through ASWB and NAADAC. 12 hours of continuing education credit will be awarded upon completion of this course through NBCC. Certificates will be emailed directly to participants roughly two weeks after the completion of the final webcast.

In order to fully complete this course and receive a continuing education certificate from one of the following boards, you must pass each of the eight weekly quizzes with an 80% or higher (you will have unlimited retests) along with the final evaluation. The required posttest questions will be at the end of each week's section and the required evaluation will be provided at the completion of the course. Upon successful completion of this course, you can request a certificate using the certificate request form posted with the evaluation and on your learning page. C4 Innovations will then review your course completion and email you a certificate within 2-3 weeks in PDF format. If you are social worker, you will need to write in your license number in the certificate request form for us to be able to include it in your certificate.
